The Medicare Pricing Pivot (January 1, 2026)
January 1, 2026, marked a historical turning point for pharmacy economics. For the first time, Medicare has implemented direct price negotiations for a list of 10 high-impact medications, including several widely used blood thinners and diabetes drugs.
While this lowers out-of-pocket costs for patients, it creates a "reimbursement squeeze" for local pharmacies.
Because pharmacies are reimbursed at the Maximum Fair Price (MFP) plus a small fee, they must now operate with razor-thin margins. This is forcing many pharmacies to shift their business model away from profit-on-product and toward Service-Based Revenue, where they get paid for the clinical advice they provide rather than the pills they sell.
